DAYTON, OH – BALLARD, Leslie Ann, age 36 of Yellow Springs, passed away unexpectedly on September 9, 2019. She was born on April 17, 1983 to proud parents Carol J. Van Loon and Donald F. Ballard, Jr. Leslie graduated from Patterson High School in 2002. She also attended Northmont High School for part of her schooling. Leslie was an outgoing and fun-loving woman. She dedicated her life to the service of others, working as a waitress at Bob Evans.

While Leslie will be remembered for her fun-loving spirit, her compassionate heart, and contagious laugh; she will above all be remembered for her love and devotion to her daughter, Samantha. Leslie found great pride in her daughter and enjoyed spending time with those she loved.

Leslie was a loving, caring, and compassionate mother, daughter, sister, aunt, and dear friend. She will be greatly missed and lovingly remembered by all who knew her.

Leslie was preceded in death by her paternal grandparents, Frances and Donald F. Ballard, Sr. and maternal grandparents, Thomas and Sarah Waits.

Leslie will be lovingly remembered by her daughter, Samantha Ballard; mother and step-father, Carol and Nick Van Loon; father, Donald F. Ballard, Jr.; brother, Anthony “Tony” Ballard; sisters, Heather (Bob) Allen and Gwen (fiance, Craig) Ballard; boyfriend, Chris; nieces and nephews, Sarah, Chris, Brian, Chelsea, Brayden, Aiden, and Madison; great-niece and nephew, Freya and Tate; and many other loving relatives and dear friends.

A funeral service honoring Leslie’s life will be held on Monday, September 16, 2019 at Fairview Brethren in Christ Church (750 Union Blvd, Englewood, OH 45322) at 6:30pm with Pastor Mark Ballard officiating. Visitation for one hour prior (5:30pm – 6:30pm).
